A home Inherited by a passed on loved one where Mrs. Middleton grew up, is now the foundation and location of Crummey's Residential Care & Boarding Home.

  I decided to dedicate my time & pledge my efforts to establish a place of refuge & shelter for other veterans and persons with mental & physical disabilities. It is of the utmost importance that I advocate for those in need and make them feel like they reside in a loving home with family values. With due diligence, hard work, humility, and everlasting faith & trust in God, my dreams of being a passionate caregiver & successful business owner will only continue to flourish as the Lord’s will prevails...

CHECKLIST

CRUMMEY’S BOARDING HOME REQUIRES A MANDATORY
FACE TO FACE APPOINTMENT FOR ALL APPLICABLE CLIENTS.

 
CONTACT OUR STAFF TO SCHEDULE
ONCE REFERRAL IS SUBMITTED.

1. Patients must be Ambulatory. (Assist x1 is max level we will accept R/T gait/transfer status).

 

2. Patients must be able to perform ALL Self Hygiene/ADLs with MINIMAL assistance.

  Incontinence is acceptable within reason depending on the patient situation and medical history.

 

*** All mobility/hygiene categories and self performance capabilities will be discussed/assessed in detail

     upon initial evaluation per staff via phone/on-site facility visit prior to final acceptance/rejection.

- We accept Mental Health Patients & Mild Dementia Diagnoses;

  NO complex cases of Alzheimer’s; AMS or Aggressive/Violent Behavioral Disorders.

 

- NO oxygen dependent patients permitted.

 

- NO terminal hospice patients permitted.

  HX of CA is permissible on a client specific basis upon evaluation per staff.

   NO chemo Patients permitted.

 

- Patients with HOME HEALTH/PT/OT/ST adjunct services permitted.

 

- NO patient with a PEG tube/Tracheostomy/Central Line

(unless for HD) will be permitted.

 

- PICC lines/Drains/Wound VACs are permitted upon client specific basis.

ALL REFERRALS WILL BE CONSIDERED AND OPEN TO DISCUSSION
IF THE FIT IS RIGHT FOR OUR HOME AND OTHER CLIENTS.
